How to get a case manager

Medicaid/Medicare

en/members-applicants/get- started/how-to-apply.html - I forgot to mention: If your loved one has a serious or chronic diagnosis and requires medication and immediate insurance coverage, ask one of your providers to fill out an emergency processing form to send in with your Medicaid application. It takes about 10 days instead of 30 for approval.

SNAP/General Assistance
- Both benefits use the same application and you can apply for both at one time. If your loved one lives in a group home or similar, do not include the other household renters, they are applying as an individual.

SSI/SSDI
- SSI does not require a work history
